Arthritis is one of the main issues that people use cannabidiol (CBD), and it is also one of the most researched areas regarding CBD. Below are summaries of two studies regarding CBD and arthritis.

This study from the National Institutes of Health (NIH) examines the impact of CBD on the paw of rats. The point of the study was to examine the anti-inflammatory and anti-hyperalgesic effects of CBD, given once a day for three days, after the onset of acute inflammation. Prostoglandin E2 (PGE2) was assayed in the plasma, and cyclooxygenase (COX) activity, production of nitric oxide, and other oxygen-derived free radicals in the inflamed paw tissues.

All these markers were increased following carrageenan. Cannabidiol had a time and dose-dependent anti-hyperalgesic effect after just a single injection. Edema peaked at three hours and lasted 72 hours. The study says:

“a single dose of cannabidiol reduced edema in a dose-dependent fashion and subsequent daily doses caused further time- and dose-related reductions. There were decreases in PGE2 plasma levels, tissue COX activity, production of oxygen-derived free radicals, and NO after three doses of cannabidiol. The effect on NO seemed to depend on a lower expression of the endothelial isoform of NO synthase. In conclusion, oral cannabidiol has a beneficial action on two symptoms of established inflammation: edema and hyperalgesia.”

A second study examines the impact of transdermal/topical CBD on arthritis. Again, plasma was measured after administering as little as 0.6mg/day. “Transdermal CBD gel significantly reduced joint swelling, limb posture scores as a rating of spontaneous pain, immune cell infiltration and thickening of the synovial membrane in a dose-dependent manner” said the study. Finally, the conclusion of the study was, “These data indicate that topical CBD application has therapeutic potential for relief of arthritis pain-related behaviours and inflammation without evident side-effects.”

Yankel Gabet, one of the researchers said, “We found that CBD alone makes bones stronger during healing, enhancing the maturation of the collagenous matrix, which provides the basis for new mineralization of bone tissue. After being treated with CBD, the healed bone will be harder to break in the future.”

A team of experts inflicted mild femoral fractures on rats, and then gave an injection of CBD following the break. Gabet said, “We found CBD alone to be sufficiently effective in enhancing fracture healing. Other studies have also shown CBD to be a safe agent, which leads us to believe we shoul continue this line of study in clinical trials to assess its usefulness in improving human fracture healing.”

Previously, this same research group found that the body has cannabinoid receptors that are not limited to the brain. Some of these receptors, trigger bone formation and prevent bone loss. This second study only confirmed these findings. Gabet said:

“We only respond to cannabis because we are built with intrinsic compounds and receptors that can also be activated by compounds in the cannabis plant. The clinical potential of cannabinoid-related compounds is simply undeniable at this point. While there is still a lot of work to be done to develop appropriate therapies, it is clear that it is possible to detach a clinical therapy objective from the psychoactivity of cannabis. CBD, the principal agent in our study, is primarily anti-inflammatory and has no psychoactivity.”

The article also points out that CBD is now been researched and shown to suppress seizures, inhibit the metastasis of many aggressive cancer, and destroy leukemia cells. The federal government still insists that there is “no currently accepted medical use”, however a recent FDA ruling may change that, because it approved the use of CBD extracts as an experimental treatment for Dravet syndrome – a rare form of childhood epilepsy.

The endocannabinoid system was discovered in 1992 by Dr. Raphael Mechoulam and researchers William Devane and Dr. Lumir Hanus. Although it was discovered in the early 90’s, it wasn’t clear what role it played in our bodies until the last decade. Studies are now emerging, with amazing evidence showing what it’s role in our bodies. Anxiety is one area where it’s role is very important.

The modulation of the Endocannabinoid System (ECS) influences a wide range of functions. These functions range from metabolic to mental disorders. This study by the National Institutes of Health explains the ECS and its role in anxiety. As the study says:

“The recently developed genetic tools have revealed different but complementary roles for the cannabinoid type 1 (CB1) receptor on GABAergic and glutamatergic neuronal populations. This dual functionality, together with the plasticity of CB1 receptor expression, particularly on GABAergic neurons, as induced by stressful and rewarding experiences, gives the ECS a unique regulatory capacity for maintaining emotional homeostasis.”

The article elaborates and explains that ECS activation deteriorates reconsolidation, but to achieve proper extinction depends on intact CB1 receptor signaling. The study concludes with this statement: “Thus, both for anxiety and fear memory processing, endocannabinoid signaling may ensure an appropriate reaction to stressful events. Therefore, the ECS can be considered as a regulatory buffer system for emotional responses.”

This article explains how to activate the ECS. Although there are many ways to activate the ECS, this article specifically points out several cannabinoids and terpenes. To many of us, epilepsy, Dravet Syndrome (DS), and seizures may be the same thing. While they all involve seizures, many of them are spurred, or are of varying degrees of seizures. Epilepsy is defined as “a neurological disorder marked by sudden recurrent episodes of sensory disturbance, loss of consciousness, or convulsions, associated with abnormal electrical activity in the brain.” DS, on the other hand, is a type of epilepsy previously known as “severe myoclonic epilepsy of infancy” – these seizures are often triggered by hot temperatures or fever.

 

In 2015, the American Epilepsy Society conducted a study examining CBD’s impact on seizures – most patients were subject to DS. For four weeks before the study, parents/caregivers kept prospective seizure diaries of all countable motor seizure types. For twelve weeks, they were monitored, and seen approximately every three weeks. They were tested for hematologic, liver, kidney function, and AED levels. Of the 261 patients in the study, 17% had DS, and 15% had Lennox-Gastaut Syndrome (LGS).

 

After twelve weeks of treatment, the median seizure reduction was 45.1%, but over 62% in patients suffering from DS. A reduction of over 71% was seen in those with LGS. Of all patients, 47% saw a reduction of at least half of their seizures. After the 12 week study, 9% of overall patients were completely seizure free, and 13% of DS patients were seizure free.  The main side effects were diarrhea, fatigue, and increased appetite.

 

The conclusion from the American Epilepsy Society said: “These results from an uncontrolled study support the animal studies and prior reports showing that CBD may be a promising treatment for TRE and it is generally well-tolerated in doses up to 25mg/kg/day. Epidiolex is now being investigated in randomized controlled studies in DS and LGS.”
